オープンキャンパスへの参加申込は、下記のフォームに必須事項をご入力のうえ、送信してください。
オープンキャンパス当日、入試に関する個別相談、および教員との面談を希望する場合は、フォームの該当欄にご記入ください。
このフォームで申込まれた内容は直接入試担当者に届きます。
また、返信はメールにて行いますので下記の注意事項に留意ください。
To register for the Open Campus, please complete the required fields in the form below and submit it.
If you would like to have an individual consultation regarding admissions and/or meet with a faculty member on the day of the Open Campus, please indicate this in the relevant section of the form.
The information submitted through this form will be sent directly to the admissions staff.
As all responses will be sent by email, please read the notes below carefully.

お申込の前に端末のドメイン設定をご確認ください。
※申込後、受付完了の返信メールをお送りします。返信メールの到着をもって受付が完了となりますので、事前に「opencampus★minpaku.ac.jp（★印を＠に変更して送信ください。）」のアドレスが受信できるよう、各端末の受信設定をしてください。
返信メールが届かない場合は、各端末の迷惑メールフィルターの設定をご確認のうえ、再度ご送信ください。
